What Is Obstetrics?

Obstetrics is the medical specialty concerned with pregnancy, childbirth, and the weeks immediately following delivery, taking both the pregnant patient and the fetus as its subjects. It is usually practiced together with gynecology as a combined specialty, and it draws on physiology, endocrinology, surgery, and imaging in roughly equal measure. Its defining feature is that two patients are managed at once, with interventions that benefit one sometimes carrying risk for the other, which is why obstetric practice depends heavily on measurement and continuous monitoring.

The specialty is organized around three periods. Antenatal care spans conception to the onset of labor, intrapartum care covers labor and delivery, and postpartum care extends through the puerperium as maternal physiology returns toward its pre-pregnancy state. Maternal-fetal medicine is the subspecialty devoted to high-risk pregnancy, and it overlaps with neonatology at the point of birth.

Antenatal Care and Diagnostics

Antenatal care is a schedule of visits designed to detect treatable conditions before they cause harm. Routine work includes confirming gestational age, tracking blood pressure and weight, screening for anemia, infection, blood group incompatibility, and gestational diabetes, and assessing fetal growth. Obstetric ultrasound is the central imaging technology: it dates gestation from crown-rump length in the first trimester, surveys fetal anatomy around 18 to 22 weeks, and uses Doppler measurement of umbilical and middle cerebral artery flow to assess placental function. Screening for chromosomal conditions has shifted substantially toward analysis of cell-free fetal DNA in maternal plasma, with invasive amniocentesis or chorionic villus sampling reserved for confirmation.

Labor, Delivery, and Monitoring

Labor is conventionally divided into three stages: cervical dilation, expulsion of the fetus, and delivery of the placenta. Electronic fetal monitoring, introduced in the 1960s, records the fetal heart rate against uterine contractions using Doppler ultrasound and a tocodynamometer, and clinicians classify the resulting tracings by baseline rate, variability, and the timing of decelerations. Obstetric Emergencies

A small set of conditions accounts for most severe maternal morbidity, and each has a defined management pathway. Postpartum hemorrhage is treated with uterotonic drugs, uterine massage, tamponade balloons, and surgical measures when those fail. Preeclampsia and eclampsia, hypertensive disorders unique to pregnancy, are managed with antihypertensives, magnesium sulfate for seizure prophylaxis, and delivery as the definitive treatment. Obstructed labor, sepsis, amniotic fluid embolism, and shoulder dystocia round out the list. Cesarean section is the principal surgical intervention, and international attention has focused on rates that are too low in some regions and higher than clinically justified in others.
