What Are Mental Disorders?
Mental disorders are clinically significant disturbances in an individual's cognition, emotional regulation, or behavior that are associated with distress or with impairment in personal, social, or occupational functioning. They are not transient responses to everyday stress but persistent patterns that deviate from culturally expected norms and that interfere meaningfully with daily life. As defined by the World Health Organization, mental disorders arise from a complex interaction of biological, psychological, social, and environmental factors rather than from any single cause.
The study and treatment of mental disorders falls within psychiatry, clinical psychology, and neuroscience, and they intersect with biomedical engineering through applications in diagnostics, computational psychiatry, and the development of brain-computer interfaces and wearable monitoring systems. According to the WHO fact sheet on mental disorders, approximately one in seven people worldwide, around 1.1 billion individuals, live with a mental disorder, with anxiety and depression being the most prevalent.
Classification and Diagnostic Systems
The two primary frameworks for classifying mental disorders are the Diagnostic and Statistical Manual of Mental Disorders (DSM), maintained by the American Psychiatric Association, and the International Classification of Diseases (ICD), maintained by the World Health Organization. The DSM-5, released in 2013 with a text revision in 2022, organizes diagnoses into chapters defined by shared symptom patterns and dimensional specifiers rather than purely categorical boundaries. The ICD-11, implemented in 2022, covers mental disorders as one chapter among all health conditions and is designed for use across clinical settings in both high-income and low-resource environments. Both systems rely on operationalized symptom criteria rather than biological markers because reliable biomarkers for most psychiatric conditions remain elusive. Major Disorder Categories
DSM-5 and ICD-11 both recognize several major categories. Anxiety disorders, including generalized anxiety disorder, panic disorder, and phobias, are defined by excessive and persistent fear or worry affecting approximately 359 million people globally. Depressive disorders, most prominently major depressive disorder, involve persistent low mood, reduced energy, and anhedonia lasting at least two weeks. Bipolar and related disorders are characterized by alternating depressive and manic or hypomanic episodes. Psychotic disorders, including schizophrenia, involve hallucinations, delusions, and disorganized thinking. Neurodevelopmental disorders, such as autism spectrum disorder and attention-deficit/hyperactivity disorder (ADHD), manifest during early development and affect cognitive and social functioning. Eating disorders, trauma- and stressor-related disorders, and substance use disorders complete the core categories addressed in both systems.
Biological and Social Determinants
Mental disorders do not have single causes. Genetic vulnerability interacts with early-life adversity, social circumstances, and neurobiological factors to determine whether a disorder emerges. Structural and functional neuroimaging studies have identified correlates of major depressive disorder in prefrontal-limbic circuits and of schizophrenia in cortical thickness and dopaminergic signaling, though no imaging finding yet serves as a diagnostic criterion. The Research Domain Criteria (RDoC) framework, developed by the US National Institute of Mental Health, aims to ground psychiatric classification in neuroscience by organizing research around dimensions of biological function such as threat responsivity, reward learning, and cognitive control, independent of existing diagnostic categories.
