Gastritis
What Is Gastritis?
Gastritis is inflammation of the gastric mucosa, the innermost lining of the stomach, confirmed histologically by the presence of inflammatory cells in a mucosal biopsy. Clinicians separate it from gastropathy, in which the lining is damaged without significant inflammation, and from peptic ulcer disease, in which the mucosal break extends deeper into the stomach wall. Cases are described as acute when onset is sudden and short-lived, and chronic when the inflammation persists for months or years and progressively remodels the lining. The NIDDK definition of gastritis and gastropathy sets out this distinction and the diagnostic criteria used in practice.
Chronic gastritis matters clinically because of where it leads. Long-standing inflammation can progress through atrophy of the gastric glands to intestinal metaplasia and dysplasia, a sequence associated with gastric adenocarcinoma, so grading and staging systems such as OLGA and OLGIM exist to quantify risk from biopsy samples. Gastritis is consequently a frequent target for imaging, sensing, and computational diagnostic work in biomedical engineering.
Causes and Classification
Infection with Helicobacter pylori is the most common cause worldwide. The bacterium colonizes the mucus layer, resists gastric acid through urease activity, and provokes an infiltrate of neutrophils, lymphocytes, plasma cells, and macrophages in the lamina propria. A review of H. pylori-induced chronic gastritis and gastric cancer risk traces that inflammatory cascade and its link to mucosa-associated lymphoid tissue lymphoma. Other causes include autoimmune destruction of parietal cells, which produces vitamin B12 deficiency and pernicious anemia, prolonged use of nonsteroidal anti-inflammatory drugs, heavy alcohol use, bile reflux, radiation exposure, and physiological stress from critical illness. The Sydney system, updated in 1994 and refined since, classifies cases by topography (antral, corporal, or pangastritis), morphology, and etiology.
Endoscopic Diagnosis and Imaging
Upper gastrointestinal endoscopy with targeted biopsy remains the reference standard. Modern instruments add optical techniques that improve the visibility of mucosal change: narrow band imaging restricts illumination to blue and green wavelengths that are absorbed by hemoglobin, sharpening the appearance of microvascular and pit patterns, while magnifying endoscopy and confocal laser endomicroscopy resolve structures near the cellular scale. Wireless capsule endoscopy provides a non-invasive alternative for parts of the tract, though the stomach's volume complicates its use there. Non-endoscopic testing includes the urea breath test, which measures labeled carbon dioxide after a dose of isotope-tagged urea, stool antigen assays, and serum pepsinogen measurement as a marker of glandular atrophy.
Computational Detection and Assessment
Automated interpretation of endoscopic video is an active engineering problem, because visual grading of atrophy and intestinal metaplasia varies considerably between observers. Convolutional network architectures such as ResNet and EfficientNet, sometimes combined with segmentation networks that first isolate the mucosal region, have been trained on tens of thousands of labeled images. A multicenter diagnostic trial of deep learning classification of gastritis reported accuracy near 89 percent for separating atrophic from non-atrophic gastritis and around 94 percent for identifying erosion and hemorrhage.
