What Is Eczema?

Eczema is a group of chronic inflammatory skin conditions marked by itching, redness, dryness, and recurrent rash, of which atopic dermatitis is by far the most common form and the one the term usually denotes in clinical use. The disease follows a relapsing course of flares and quiescent periods, typically begins in infancy, and affects roughly 10 to 20 percent of children and 2 to 10 percent of adults in industrialized countries. It is not contagious. The National Institute of Arthritis and Musculoskeletal and Skin Diseases characterizes it as a long-lasting disease causing inflammation and irritation of the skin, with intense itch as the defining symptom. The distribution of the rash shifts with age, appearing on the face and scalp in infants, in the elbow and knee flexures in children, and on the hands, neck, and eyelids in adults.

Eczema sits at the intersection of dermatology, immunology, and biomedical instrumentation. Its severity is judged by visible signs and by patient-reported itch, both of which are difficult to quantify reproducibly, which has made the condition a recurring target for sensing and imaging research.

Skin Barrier Dysfunction and Immune Mechanisms

The pathophysiology combines a defective epidermal barrier with a skewed immune response. The stratum corneum in affected skin shows reduced levels of filaggrin, a structural protein that aggregates keratin filaments and supplies natural moisturizing factor; loss-of-function mutations in the filaggrin gene are the strongest known genetic risk factor for atopic dermatitis. A weakened barrier raises transepidermal water loss and lets allergens, irritants, and microbes penetrate, which drives type 2 inflammation dominated by interleukin-4, interleukin-13, and interleukin-31. Those cytokines further suppress barrier protein expression and sensitize cutaneous nerve fibers, so scratching damages the barrier again. Reviews of the etiopathogenesis of atopic dermatitis describe this barrier, immune, and itch triad as a self-reinforcing loop rather than a linear sequence. Colonization by Staphylococcus aureus, present on most lesional skin, aggravates the cycle.

Diagnosis and Severity Measurement

Diagnosis is clinical, based on morphology, distribution, chronicity, and personal or family history of atopy, using criteria such as those of Hanifin and Rajka or the UK Working Party. Severity is scored with instruments including SCORAD, the Eczema Area and Severity Index, and the Patient-Oriented Eczema Measure, each of which combines lesion extent, intensity of signs, and subjective symptoms. Inter-rater variability in these visual scores is substantial, and the clinical overview of eczema in StatPearls notes how much the assessment depends on examiner judgment. Instrumental measures address part of the gap: evaporimetry quantifies transepidermal water loss, corneometry measures hydration, optical coherence tomography and high-frequency ultrasound image epidermal thickness, and digital image analysis grades erythema without a human scorer.

Sensing, Monitoring, and Treatment

Itch is the symptom patients rank as most burdensome, and nocturnal scratching is the behavioral trace of it. Wrist-worn accelerometers, piezoelectric sensors, electromyography, and touchless radar have all been applied to detecting scratch events during sleep, with classifiers trained against video-scored ground truth; work on machine learning quantification of nocturnal scratch using wrist actigraphy illustrates both the approach and the difficulty of separating scratching from ordinary movement. Treatment remains stepwise: emollients and trigger avoidance for maintenance, topical corticosteroids and calcineurin inhibitors for flares, and for moderate to severe disease, phototherapy, biologics targeting interleukin-4 and interleukin-13 signaling, or Janus kinase inhibitors.
