What Is Chronic Obstructive Pulmonary Disease?

Chronic obstructive pulmonary disease, universally abbreviated COPD, is a preventable and treatable lung condition characterized by persistent respiratory symptoms and airflow limitation that is not fully reversible, arising from abnormalities of the airways or the alveoli after prolonged exposure to noxious particles or gases. Tobacco smoke is the dominant cause in high-income countries, while indoor biomass fuel smoke, occupational dust and fume exposure, and ambient air pollution account for a large share of cases elsewhere. A minority of patients carry alpha-1 antitrypsin deficiency, an inherited protease inhibitor defect that produces early-onset emphysema.

COPD is not a single lesion but a combination of two processes that coexist in varying proportion. Obstructive bronchiolitis narrows and scars the small airways below two millimeters in diameter, raising resistance. Emphysema destroys alveolar walls, which reduces the surface area available for gas exchange and, just as importantly, removes the elastic tethers that hold small airways open during exhalation. Both mechanisms slow expiratory flow, trap air, and force the lungs to operate at a hyperinflated volume where the diaphragm works at mechanical disadvantage.

Diagnosis and Spirometry

Diagnosis requires spirometry, which records the volume and flow of a forced exhalation. The clinically important quantities are forced vital capacity, the total volume expelled, and forced expiratory volume in one second, the volume expelled during the first second. The Global Initiative for Chronic Obstructive Lung Disease defines obstruction as a post-bronchodilator ratio of those two below 0.70, and grades severity by the percentage of predicted forced expiratory volume in one second. The fixed 0.70 threshold is convenient but contested, since the ratio falls with age in healthy people, so the criterion overdiagnoses older adults and underdiagnoses younger ones. An alternative uses the lower limit of normal, the fifth percentile of a reference population matched for age, sex, height, and ancestry. A comparison of spirometric criteria in the Burden of Obstructive Lung Disease study quantified how much prevalence estimates shift between the two definitions, and later work on the discriminative accuracy of ratio thresholds for hospitalization and mortality tested which cutoff best predicts hard clinical outcomes. Measurement quality is a real constraint on all of this: spirometers must meet accuracy and repeatability specifications set jointly by the American Thoracic Society and the European Respiratory Society, and results depend heavily on patient effort and coaching, which is why automated quality grading is built into modern devices.

Exacerbations and Disease Course

Lung function declines gradually, but the trajectory is punctuated by exacerbations, acute periods of worsened symptoms usually triggered by viral or bacterial infection or by pollution exposure. Exacerbations drive hospital admission, accelerate the loss of function, and predict mortality, so much of clinical management aims at preventing them rather than at reversing established obstruction. Patients are stratified by spirometric grade and also by symptom burden, using instruments such as the modified Medical Research Council dyspnea scale and the COPD Assessment Test, and by exacerbation history over the prior year. Cardiovascular disease, skeletal muscle wasting, osteoporosis, and depression are common comorbidities that shape outcomes independently of lung mechanics.

Management and Supporting Technology

Smoking cessation has the greatest effect on the natural history of the disease and is the intervention with the clearest evidence for slowing the decline in lung function. Pharmacologic treatment relies on inhaled long-acting muscarinic antagonists and beta-2 agonists, with inhaled corticosteroids added for patients with frequent exacerbations and eosinophilic inflammation, all delivered through metered-dose, dry powder, or nebulized devices whose aerosol particle size distribution determines where the drug deposits. Pulmonary rehabilitation improves exercise capacity and quality of life. Long-term oxygen therapy, delivered by pressure swing adsorption concentrators, extends survival in patients with resting hypoxemia, and non-invasive ventilation supports those with chronic hypercapnia. Selected patients benefit from lung volume reduction, either surgically or by bronchoscopically placed one-way endobronchial valves, and quantitative computed tomography is used to measure emphysema extent and fissure integrity before those procedures.
