Anemia
What Is Anemia?
Anemia, spelled anaemia in British usage, is a condition in which the concentration of hemoglobin in the blood, or the number of circulating red blood cells, falls below the level needed to meet the body's demand for oxygen. It is defined by measurement rather than by symptoms, with diagnostic thresholds that vary by age, sex, pregnancy status, altitude, and smoking history. The World Health Organization estimates that roughly 40 percent of children aged 6 to 59 months, 37 percent of pregnant women, and 30 percent of women aged 15 to 49 are affected, which makes it one of the most widespread conditions in global health.
Because hemoglobin carries oxygen from the lungs to tissue, a shortfall produces fatigue, breathlessness on exertion, pallor, tachycardia, and reduced work capacity, and in children it is associated with impaired cognitive development. Anemia is a sign rather than a diagnosis in itself: identifying the underlying cause is the clinical task, and the measurement problem, how to determine hemoglobin quickly, cheaply, and at scale, is the engineering one.
Classification and Causes
Clinical classification usually starts from red cell size, reported as mean corpuscular volume in a complete blood count. Microcytic anemia, with small cells, points toward iron deficiency or a thalassemia trait. Macrocytic anemia, with large cells, points toward vitamin B12 or folate deficiency. Normocytic anemia covers acute blood loss, hemolysis, chronic kidney disease with reduced erythropoietin production, and the anemia of inflammation. A second axis separates causes by mechanism: decreased production in the marrow, increased destruction of circulating cells, and loss through bleeding. Dietary iron deficiency remains the single largest contributor worldwide, followed by inherited disorders such as sickle cell disease and thalassemia, malaria in endemic regions, and blood loss from menstruation, childbirth, and parasitic infection.
Measurement and Laboratory Diagnosis
The reference measurement is a complete blood count performed on an automated hematology analyzer, which counts and sizes cells using impedance or optical light scatter and reports hemoglobin concentration by a colorimetric method, typically a cyanide-free derivative assay read at a fixed wavelength. Follow-up testing distinguishes causes: serum ferritin and transferrin saturation for iron status, reticulocyte count for marrow response, peripheral smear morphology for hemolysis and abnormal cell shapes, and hemoglobin electrophoresis or high-performance liquid chromatography for hemoglobinopathies. Point-of-care devices extend testing beyond the laboratory, using single-use cuvettes with photometric readers or hand-held analyzers that need only a fingerstick sample, at the cost of somewhat wider measurement error.
Noninvasive and Computational Screening
Screening large populations with venipuncture is slow and expensive, which has driven work on optical methods that estimate hemoglobin without drawing blood. Pulse co-oximetry extends conventional pulse oximetry with additional wavelengths to estimate total hemoglobin at the fingertip. Camera-based approaches analyze color in tissue where capillary beds sit close to the surface, and a widely cited study in Nature Communications on smartphone detection of anemia from patient-sourced fingernail photographs demonstrated the approach with per-person calibration to improve accuracy on repeat measurements. Later systems apply machine learning to images of the lower eyelid conjunctiva, and a clinical validation of a smartphone application estimating blood hemoglobin in real time reports the agreement achieved against laboratory reference values. Accuracy under varied skin tone, ambient lighting, and camera hardware remains the limiting factor for deployment.
